“Always Finish Your Antibiotics” — Is This Advice Actually Wrong?

You’ve heard it a thousand times. From your doctor, your parents, that pharmacist who barely looked up from the counter: “Finish the full course, even if you feel better.”

But what if this golden rule of medicine… isn’t entirely golden?


What the New Science Says

A landmark review in The BMJ shook the medical world. The conclusion? The idea that stopping antibiotics early drives resistance is not supported by evidence. In fact, taking antibiotics longer than necessary may actually increase resistance — because extra days expose harmless gut bacteria to the drug, giving them more time to develop and share resistance genes.

The evidence for common infections is striking:

  • Pneumonia: 5 days works as well as 10 in most cases
  • UTIs: 3-day courses match longer treatments
  • Sinusitis: 3–7 days matches 10–14 day courses

Even the WHO quietly dropped the “always finish your course” message in 2017. Most people never noticed.


So Should You Just… Stop?

Not so fast. This isn’t a free pass to toss your pills the moment you feel better. Some infections — like tuberculosis or bone infections — absolutely require the full prescribed course. The real issue? Many standard prescriptions were set at 10 or 14 days out of habit, not evidence.


What You Should Actually Do

Talk to your doctor. Not Dr. Google, not leftover pills from last winter. Ask specifically: “How long do I actually need this?”

A good doctor today will prescribe the shortest effective course based on current evidence — and tell you to check back if symptoms improve early.

This matters more than you think. India has among the highest antibiotic resistance rates in the world, with nearly 300,000 deaths linked to resistant infections annually. Every unnecessary day of antibiotics makes this worse — for everyone.


The Bottom Line

“Always finish the course” was well-intentioned advice. But medicine evolves. The new message is smarter: take antibiotics for exactly as long as you need them — no more, no less.

Your doctor decides that duration. Not tradition. Not habit. Not fear.
